Venous pulsations are best assessed by inspection of the right internal jugular vein (IJV). As the diagram on the right demonstrates, the right IJV has a direct course to the right atrium and is free of valves, making it a good barometer of pressure and hemodynamic events in the right atrium. In general, the left internal jugular vein and both external jugular veins are less reliable for assessing venous pressure and pulsations. The video demonstrates an engorged right external jugular vein without obvious pulsations. On careful inspection, however, venous pusations are seen just below the ear at the end of the video.
